Ellwood City Uni-Center 718-720 Lawrence Ave., Ellwood City HSC utilizes this space to provide services to residents of the greater Ellwood City area. Office space is also leased to other social service providers. Sixteen SRO (Single Room Occupancy) housing units are also located in this building.

Washington Street Apartments 1026 E. Washington St., New Castle Eight apartments are available for HSC clients who need permanent, independent housing.

Oakview Apartments 7 S. Liberty St., New Castle A ten-unit apartment building that is funded by HUD and is utilized by chronically mentally ill adults.

North Street Apartments 20-24 E. North St., New Castle Fourteen studio apartments for adults with a history of mental illness and/or a developmental disability.

Westfield 5826 Old Pulaski Rd., New Wilmington An eight-bed group home for adults with a developmental disability. Funded through HUD's Section 811 program and licensed as a Community Rehabilitation Residence (CRR) and a Personal Care Home (PCH).

Edgewood 5960 Old Pulaski Rd., New Wilmington A nine-bed LTSR that provides short-term residency and intensive treatment for adults with an acute and or chronic mental illness.

Leaf Recycling Facility Old Pulaski Rd., New Wilmington An indoor site for vocational rehabilitation services including the recycling of leaf waste.

Horizon House 24 E. Grant St., New Castle A group home for eight chronically mentally ill adults and licensed as a Community Rehabilitation Residence (CRR) and a Personal Care Home (PCH).

Fairweather Lodge 442 Croton Ave., New Castle A residential home for up to six adults with a history of homelessness and/or mental illness.

Long-term Permanent Housing 822 Butler Ave., New Castle Two one-bedroom apartments for single adults.

Transitional Housing 510 Centennial St., New Castle A shared housing complex providing short or long-term care for up to six homeless adults.

Pearson Street CRR 232 Pearson St., New Castle A fully supervised group home for eight adults with chronic mental illness, licensed as a CRR.

Fairweather Lodge 111/115 E. Grant St., New Castle A residential home for up to eight adults with a history of homelessness and/or mental illness. The top floor of 115 East Grant provides office space for residential staff as well as Mobile Medication staff.

Highland Cooperative 301 Highland Ave., New Castle Located on the first floor is a three bedroom facility that provides transition housing for adults with mental illness. Six studio apartments are on the second and third floors.

Administrative Offices 302 North Mercer St., New Castle Early Intervention and Student Assistance Program staff are located on the second floor. HSC's community services department is on the third floor. (Eckles Architecture leases office space on the first floor.)

Caritas Residential Treatment Facility 2882 Princeton Rd., Slippery Rock Township A fully supervised nine-bed Residential Treatment Facility for 12-18 year old adolescent females.
