TRANSITIONAL AFTERCARE PROGRAM

The Transitional Aftercare Program (TAP) is a parent/guardian’s link to the follow-up services their child may need after being released from an inpatient or partial hospitalization setting. Parents/Guardians will be informed of treatment options and other services available in this area. A TAP Counselor can provide short-term, supportive counseling to the child/adolescent, if necessary, until their scheduled appointment.

Eligibility
To be eligible for the Transitional Aftercare Program you must be a child/adolescent currently residing in Lawrence County who:
·     is being discharged, or has recently been discharged from an inpatient psychiatric facility; 
·     who was assessed for inpatient services and was not admitted but is in need of further
treatment options; or
·     who is being or has recently been discharged from any area partial hospitalization program.


Program Funding
Services for all Lawrence County children and adolescents are free. Funding for this program is provided through the Lawrence County Office of Mental Health/Mental Retardation (MH/MR).

Program Staff
The staff is comprised of professionals in behavioral health care. The coordinator of the program is a licensed M.S.W. with more than 20 years of experience in the mental health field. A board certified child psychiatrist provides consultation to the TAP staff on a regular basis.

Referrals
Referrals occur through the inpatient or partial hospitalization program. Parents or guardians must sign a release of information for HSC to be informed of a child’s pending discharge; or they may contact us directly.
